Qualifications

Responsibilities

• Conduct onboarding calls with patients and families.
• Build individualized care plans covering symptoms and risk factors.
• Lead monthly check-ins to assess changes and coordinate resources.
• Manage care logistics: referrals, refills, and reminders.
• Source regional resources (nutrition, exercise, social support) for patients.
• Collaborate with the tech team to shape AI-enabled care delivery.

What You’ll Do

• Conduct onboarding calls with new patients and their families
• Build individualized care plans covering symptoms, modifiable risk factors, and medical and non-medical interventions
• Lead monthly check-ins: assess changes from baseline, recommend care resources, and coordinate with caregivers and clinics
• Manage care logistics, including referral scheduling, medication refill flags, and appointment reminders
• Find and vet regional and disease-specific resources (exercise, nutrition, sleep, social engagement) for your patients
• Partner with the technology team, giving feedback on the care platform and helping shape how AI fits into patient care

What You Bring

• Active RN license (required)
• CCM/PCM experience preferred
• Strong clinical knowledge: you can comfortably explain chronic conditions, how they progress, what families should watch for, and what helps
