Job summary

You will collaborate with physicians, nurses, social workers, case managers, and coordinators to ensure seamless care, educate families, monitor immunosuppression, and coordinate pre-transplant workups.

Qualifications

• Bachelor's Degree in Nursing required.
• RN – Ohio Board of Nursing licensure required.
• Minimum 2 years transplant nursing or 5 years general nursing.
• Experience coordinating complex patient care preferred.

Responsibilities

• Coordinate pre- and post-transplant care for patients.
• Educate patients and families on transplant process.
• Collaborate with multidisciplinary team to plan care.
• Manage UNOS documentation and compliance.
• Participate in on-call rotation.
• Optimize continuity of care across inpatient and outpatient settings.
