Job summary

OhioHealth is seeking a Solid Organ Transplant Coordinator (Kidney) to coordinate evaluation and preparation for organ transplant and long-term post-transplant management across inpatient and outpatient settings.

The role requires nursing licensure (RN) with at least 2 years of transplant nursing experience or 5 years of clinical nursing background, plus strong problem solving and collaboration skills to support a high-quality continuum of care.

Qualifications

• Minimum 2 years of experience as a nurse in transplant or at least 5 years of clinical nursing experience.
• Strong understanding of transplantation care and post-transplant management.
• Ability to coordinate complex care across inpatient and outpatient settings.

Responsibilities

• Coordinate pre-transplant evaluations and education for patients and families.
• Manage post-transplant follow-up and monitor organ function and health status.
• Collaborate with physicians, nurses, social workers, and other teams for continuum of care.
• Provide immunosuppression management education and support to patients.
