Cleveland Clinic Weston Hospital is seeking a Transplant Coordinator to oversee kidney transplant activities. You will support care plans, assemble test results, coordinate transplants, and collaborate with the transplant team to ensure patient-centered care.

You will work days with on-call shifts, triage patients, and liaise with referring physicians, insurance companies, and the Organ Procurement Organization to facilitate organ procurement and transplant scheduling.

Qualifikationen

• BLS certification required through AHA or Red Cross.
• Two years of RN experience.
• One year of acute care disease-related hospital experience.

Aufgaben

• Assess patients in the outpatient clinic with transplant physician.
• Coordinate plans of care and symptom management with outpatient caregivers.
• Arrange follow-up appointments and test results with referral physicians.
• Coordinate with surgeons, organ procurement, and insurance for transplant procedures.
• Prepare cases for the Selection Committee and manage pre-transplant evaluation.
