Qualifications

• Graduate from an approved school of nursing.
• Current Ohio nursing license.
• BSN preferred.
• Prior transplant experience preferred.
• Current Certified Clinical Transplant Coordinator (CCTC) certification preferred.
• CPR certification required.

Responsibilities

• Coordinate post-discharge follow-up for transplant recipients.
• Liaise with transplant surgeons/physicians, nurses, pharmacies, laboratories, home health agencies and other support services.
• Provide patient education as indicated and support the patient and family.
• Assess patient status and collaborate with the transplant team to formulate appropriate care actions.
