Job summary

The Ohio State University Wexner Medical Center is seeking an Abdominal Post Transplant Coordinator (RN) to manage post-discharge follow-up for liver, kidney, and pancreas recipients across transplant locations.

This role requires coordination with surgeons, nurses, pharmacists, and home health services, along with patient education and responsive care planning in collaboration with the transplant team.

Qualifications

• Graduate from an approved school of nursing.
• Current Ohio Nurse License.
• BSN preferred.
• Prior transplant experience preferred.
• Current Certified Clinical Transplant Coordinator (CCTC) certification preferred.

Responsibilities

• Coordinate post-transplant follow-up care for liver, kidney and pancreas recipients.
• Act as liaison between transplant surgeons/physicians, nurses, pharmacies, laboratories, and home health agencies.
• Provide patient education as indicated.
• Assess patient status and collaborate with the transplant team to determine the appropriate course of action.

Education

Registered Nurse (RN) license in Ohio

BSN preferred

CCTC certification preferred
