Job summary

The Ohio State University Wexner Medical Center is seeking a Clinical Transplant Coordinator to coordinate post-discharge follow-up care for liver, kidney and pancreas transplant recipients. You will serve as liaison between transplant surgeons, nurses, pharmacies, labs and home health teams, and provide patient education as needed.

Responsibilities include assessing patient data, collaborating with the transplant team, and helping plan appropriate care pathways to ensure continuity and quality

Qualifications

• Must be a graduate from an approved school of nursing.
• Current Ohio nursing license.
• BSN preferred.
• Prior transplant experience preferred.
• Current Certified Clinical Transplant Coordinator (CCTC) certification preferred.
• Maintain CPR certification.

Responsibilities

• Coordinate post-transplant follow-up care after discharge.
• Act as liaison between transplant surgeons/physicians, nurses, pharmacies, laboratories, home health agencies and other support services.
• Provide patient education as indicated.
• Assess patient data and collaborate with the transplant team to determine an appropriate course of action.
