Sanford Health, the nation’s largest rural health system, seeks an RN Transplant Coordinator to lead education and coordination between the transplant center and referral sources. The role requires a nursing degree and an active RN license, with clinic hours Monday through Friday 8am-5pm and on-call expectations.

You will manage a caseload through pre/post transplant and ensure compliance with regulations, while partnering with nephrology and dialysis units to streamline referrals and patient

Qualifications

• Graduate from an accredited nursing program.
• Currently holds an unencumbered RN license.
• Minnesota RN license required within six months of date into position.

Responsibilities

• Coordinate the transplant center with referral sources for timely education and workup.
• Manage a designated caseload during pre/post transplant.
• Provide outreach to regional dialysis and nephrology programs.
