Job summary

Sanford Health in Fargo, ND seeks an RN Transplant Coordinator to coordinate transplant education and care between the center and referral sources. The role includes managing a caseload across pre/post-transplant stages and ensuring compliance with program requirements.

Work hours are Mon-Fri 8am-5pm with on-call duties; a portion of work may be done from home after training. Bachelor of nursing required; Minnesota RN license needed within six months of start.

Qualifications

• Bachelor's degree in nursing is required.
• RN license in the state of practice and multistate if applicable.
• Minnesota RN license must be obtained within six months of date into position.

Responsibilities

• Coordinate education and care for transplant patients before and after transplantation.
• Manage a designated caseload through pre/post-recipient and pre/post-donor processes.
• Coordinate referrals, address roadblocks in workup, and educate patients, families, and staff.
