Job summary

UNC Health Care in Chapel Hill is seeking a Nurse Navigator to coordinate solid organ and bone marrow transplant care across pre-, intra-, and post-transplant phases. You will act as a key liaison among patients, medical teams, and procurement organizations to ensure smooth communication and continuity of care.

The role requires a BSN and NC RN licensure with a minimum of three years' nursing experience; transplant-specific credentials are encouraged within two years.

Qualifications

• BSN is required and RN licensure in North Carolina.
• Solid organ transplant coordination experience preferred; certification within two years encouraged.
• Minimum of 3 years nursing experience in a clinical setting.

Responsibilities

• Coordinate individualized pre- and post-transplant care plans with multidisciplinary teams.
• Oversee clinical, psychological, and financial patient needs and test coordination.
• Coordinate pre-procedure tests and transplant candidacy discussions.
• Manage transplant waiting lists and organ offers for solid organ transplants.
• Facilitate transplant process by coordinating patient and medical teams; monitor status post-transplant.
• Educate patients and families about transplant processes and prepare educational materials.
