Job summary

Sanford Health in Bismarck, ND is seeking an RN Transplant Coordinator to manage the transplant process for renal patients, coordinating referrals, workups, education, and activation.

This role focuses on quality metrics, maintaining monthly scorecards, and reporting to the transplant committee, while ensuring compliance with regulatory requirements.

Eligible candidates will have an accredited nursing degree and an active RN license; Minnesota licensure within six months may be required.

Qualifications

  • Graduate from an accredited nursing program with a nursing degree.
  • Bachelor’s Degree in nursing required or in progress with expected completion within five years.
  • Current unencumbered RN license; multistate licensure may apply in NLC states.
  • Minnesota RN license required within six months of start date.

Responsibilities

  • Coordinate the transplant process for assigned patients from evaluation through post-transplant care.
  • Manage information for internal and external transplant databases.
  • Serve as case manager to ensure timely education, workups and activation for transplantation.
  • Maintain monthly scorecards and provide updates to the transplant committee.
  • Ensure compliance with governing agencies and regulatory requirements.
