Job summary

Froedtert Health is seeking a Transplant Outreach Coordinator to collaborate with referring providers and dialysis centers, driving transplant referrals and education across the region.

The role requires an RN license with 3+ years in transplant coordination, strong organizational and communication skills, and the ability to travel. This on-site position supports the SOT service line and community partnerships.

Qualifications

• Minimum of 3 years as a Transplant Coordinator is required.
• Previous outreach, marketing or sales experience is preferred.
• Organizational and communication skills; ability to work independently and take programmatic initiative.
• Knowledge of data systems, outcomes data and other clinical performance data.
• Ability to travel to locations throughout Wisconsin and Northern Illinois.
• Bilingual in Spanish is preferred.

Responsibilities

• Collaborate with referring providers and dialysis centers to generate and support transplant referrals.
• Serve as liaison between transplant team and external partners to strengthen relationships and increase referrals.
• Support SOT service line leadership with data-driven opportunities for program growth.
• Assist in outreach initiatives and community education.
