Job summary

Yale New Haven Health (YNHHS) seeks an experienced Intake Nurse to manage referrals for Home Care Services. You will review physician referrals, confirm service eligibility, and coordinate admissions with clinical managers.

Proficiency with EPIC Care Link and external sources is expected, along with strong communication across disciplines. You will enter referrals into the EMR, verify insurance details, and support monthly reporting while upholding the organization’s values of integrity,

Qualifications

• Associates or Diploma in Nursing required; BSN preferred.
• Connecticut RN license valid or eligible.
• Minimum five years of nursing experience with at least three in home health care.
• Knowledge of insurances, managed care, PDGM, OASIS, and home care eligibility; coding knowledge desirable.
• Excellent written and verbal communication; ability to work with diverse healthcare teams; strong computer skills.
• Ability to learn quickly, think critically, and manage multiple tasks.

Responsibilities

• Receives referrals from physicians, EPIC Care Link, phone, or fax.
• Interprets referrals for diagnosis clarity and service requests.
• Coordinates scheduling of admissions with nurse/therapy managers.
• Enters referrals into the EMR and maintains records.
• Verifies insurance information with clinical support.
• May prepare monthly referral activity reports for the Director, Home Care Services.
• Develops strong working relationships within YNHHS and with outside referral sources.
• Exhibits professionalism and problem-solving, teamwork and cooperation.
• Attends required meetings and completes annual requirements.
• Adheres to organizational values and patient-centered care.
