Job summary

Suncrest Hospice in Walnut Creek, CA is seeking an Intake Coordinator with an Active RN license (CA) and hospice familiarity. You will handle inquiries, referrals, chart audits, and EMR documentation while ensuring compliance and timely admissions.

The role requires strong multitasking and customer service skills, and offers a comprehensive benefits package with retirement and PTO. Schedule is Tue-Sat, 10:00 AM–6:30 PM, on-site at our Walnut Creek location.

Qualifications

• Active RN license in California
• Hospice experience preferred
• Strong communication and customer service
• Proficient with medical terminology

Responsibilities

• Answers telephone inquiries and channels them appropriately
• Assists with tracking and processing referrals
• Assist with audits and chart reviews as assigned
• Uploads documents to electronic medical records system
• Provides feedback on process and flow improvement
• Assists with scheduling goals of care and admission visits
• Work with sales team to receive incoming referral information
• Ensures the patient's chart in our EMR system and the consents are compliant
• Other duties as assigned by Director of Intake
• Audits all necessary paperwork on all new patients and ensures that they are completed in a timely manner and in accordance with policies and procedures

• Working knowledge of business management, governmental regulations, Conditions of Participation, State hospice regulations and CHAP standards.
