Job summary

Brighton/Suncrest Hospice seeks an experienced Registered Nurse (RN) to deliver compassionate hospice care in patients' homes and settings such as SNFs and assisted living facilities. You will collaborate with an interdisciplinary team to develop and implement personalized care plans.

In this role, you will manage your daily schedule independently, maintain strong documentation, and contribute to a low clinician caseload that enables meaningful time with patients and families.

Qualifications

• Current RN license in good standing in California.
• Patient-centered hospice care mindset.

• Ability to work independently while managing daily schedule.

• Comfort using EMR systems.

Responsibilities

• Provide compassionate hospice care in patients' homes and facilities.

As a Registered Nurse (RN), you'll provide compassionate, individualized hospice care wherever patients call home—including private residences, skilled nursing facilities, and assisted living communities.

You will collaborate with an interdisciplinary team of nurses, Social Workers, Chaplains, CNAs, and volunteers to develop care plans that support the physical, emotional, and spiritual needs of patients and their families. We prioritize quality over quantity by maintaining lower clinician caseloads and higher staff-to-patient ratios. This allows our nurses to spend more meaningful time with patients and families, build lasting relationships, and provide the compassionate, personalized care every patient deserves. Depending on your strengths and interests, we have various nursing opportunities available.

Qualifications

• Current Registered Nurse (RN) license in good standing in the state of practice

• A passion for providing exceptional, patient-centered hospice care

• Strong collaboration and communication skills with interdisciplinary care teams

• Ability to work independently while effectively managing your daily schedule

• Excellent time management and organizational skills to balance patient visits, documentation, meetings, and urgent needs

• Comfort learning and utilizing electronic medical record (EMR) systems

• Reliable transportation, a valid driver's license, and current auto insurance to travel throughout the service area
