Job summary

Qualifications

• RN licensure in state of employment is required.
• Weekend and holiday coverage may be required.
• Associates degree in nursing; BSN preferred.

Responsibilities

• Lead complex discharge planning and coordinate care with the healthcare team.
• Collaborate with physicians and MSW for psychosocially complex cases.
• Ensure proactive, safe discharge plans in line with policies and mission.
