Job summary

State of Colorado seeks an experienced Nurse II for the Homelake facility in Monte Vista to provide nursing and administrative oversight across shifts. You will mentor staff, supervise RN/LPN/CNA teams, and ensure high-quality patient care in a regulated environment.

Responsibilities include care plan development, health-change monitoring, and accurate EHR documentation, all while adhering to facility policies and safety standards.

Qualifications

• Current, valid licensure as a Registered Nurse with the Colorado Board of Nursing or participating state via eNLC.
• BLS or CPR certification is required and must be maintained.
• Two years of professional experience in a related nursing field.
• Background checks including CBI, FBI fingerprinting, and state databases are required.

Responsibilities

• Provide nursing and administrative oversight for the shift, supervising three or more FTEs.
• Direct, delegate, and prioritize daily nursing assignments (RN/LPN/CNA) based on resident needs.
• Mentor and coach nursing staff; ensure competencies are completed and evaluated.
• Maintain accurate resident records in the EHR with assessments and MAR/TAR documentation.
• Ensure compliance with VCLC, Colorado, and VA regulations and promote infection control practices.
