Job summary

Optimal Home Care, Inc. is seeking a Registered Nurse to provide in-home patient care in Colorado. The RN will perform visits, care coordination, and documentation, while supervising LPNs and collaborating with facility staff.

Qualifications

Responsibilities

• Perform RN visits and care coordination per patient needs.
• Complete SOC/ROC/RECERTs and supervisory visits as required.
• Document patient care timely per guidelines.
• Supervise LPNs in assigned facilities and manage visit utilization.
• Coordinate with care team and facility staff for patient needs.
