Job summary

Optimal Home Care in Colorado is seeking a Registered Nurse to provide compassionate patient care in home settings. The role involves supervising LPNs, coordinating care, and supporting census growth in home health and hospice.

You will complete required documentation per Medicare guidelines, manage patient care within RN scope, and collaborate with facility staff and marketers. A Colorado nursing license, valid driver's license, and at least one year of RN experience are required; OASIS

Qualifications

• Graduate of an accredited, approved school of professional nursing.
• Active Colorado nursing license in good standing.
• Must be able to communicate with clients and document activities appropriately.
• Must have a valid driver’s license.
• At least one year of RN experience; home health a plus.
• Familiarity with OASIS preferred.

Responsibilities

• Complete SOC/ROC/RECERTs and revisits as needed.
• Complete documentation per agency and Medicare guidelines.
• Provide RN care within scope of practice; supervise LPNs.
• Coordinate care across accounts/territory and attend reviews.
• Maintain consistent communication with facility staff.
• Support census growth with marketer; manage fiscal outcomes.
• Collaborate with marketer/account manager and hospice staff.
• Manage utilization of disciplines to meet patient needs; fiscal reporting as requested.
• Attend care coalitions (e.g. SDCC) when scheduled.
• Work hours typically a minimum of 40 hours per week, Mon–Fri.
