Job summary

Optimal Home Care is seeking a Registered Nurse in Colorado to provide skilled nursing services for our patients. The role covers SOC/ROC/RECERTs, supervision of LPNs, care coordination, and collaboration with facility staff to support patient outcomes.

Expect a minimum 40 hours per week, typically Mon-Fri, with a strong emphasis on quality patient care. The ideal candidate holds an accredited nursing degree, an active CO license in good standing, and at least 1 year of RN experience (home

Qualifications

• Graduate of an accredited, approved school of professional nursing.
• Active Colorado nursing license in good standing.
• Minimum 1 year RN experience; home health experience a plus.
• Familiarity with OASIS preferred.

Responsibilities

• Complete SOC/ROC/RECERTs, supervisory visits and revisits as needed.
• Perform RN duties within scope of practice and supervise LPNs.
• Coordinate care across accounts/territory and attend resident reviews.
• Communicate with facility staff and contribute to census growth.
• Prepare fiscal reports and collaborate with marketing and clinical staff.
