Qualifications

• Graduate of an accredited nursing program.
• At least 2 years of acute hospital case management or healthcare experience.
• Active RN license in the practice state(s).
• Preferred: Bachelor's or master's degree in nursing, 5 years of leadership in case management, ACM accreditation.

Responsibilities

• Facilitate care transitions through effective resource management and patient assessment.
• Balance clinical needs with financial considerations, advocating for patient needs while managing resources judiciously.
• Develop and implement transition plans by integrating patient assessments, preferences, and available resources.
• Identify patients at risk for readmission and initiate appropriate interventions, including referrals to Social Work or Case Review.
• Screen patients for factors affecting care progression and intervene to promote timely throughput.
• Perform related duties as required.
