UMass Memorial Health is seeking a Case Manager to coordinate and facilitate the management of care for an assigned group of patients. The role emphasizes early discharge planning, collaboration with the healthcare team, and ensuring a well-coordinated, cost-conscious care experience.

Ideal candidates hold a BSN and Massachusetts RN license, with 3–5 years of clinical experience and strong interpersonal/analytical skills. A CCM certification is preferred and a signing bonus may be offered.

Qualifications

• BSN required; Current Massachusetts RN license.
• 3–5 years of clinical experience with sound judgment and knowledge of current medical care and support systems.
• Preferred: CCM certification.
• Experience in utilization review, quality assurance, or community experience.

Responsabilités

• Assess inpatient admission appropriateness and identify high-risk patients needing referrals.
• Participate in development and monitoring of interdisciplinary guidelines and variance tracking.
• Collaborate with physicians and healthcare providers to improve quality of care and utilization management.
• Facilitate timely discharge through early intervention and coordination of services.
• Monitor compliance, collect utilization data, and identify cases for quality and utilization review.
