BayCare is hiring a Hospital Care Coordinator (RN) for the Tampa area. This PRN role supports St. Joseph's Main and Children's campuses, focused on care coordination, discharge planning, and advocacy for patients. Requires an RN license and an Associates degree in nursing, with Bachelor’s preferred.

The position emphasizes flexible scheduling, a supportive team, and opportunities to impact patient outcomes within a large health system in Tampa. PRN hours: 8:00AM–4:30PM, On-site, exempt status.

Qualifications

• RN license required.
• Associate's Degree in Nursing required; Bachelor’s degree preferred.
• ACM and CCM certifications preferred.

Responsibilities

• Provides linkage, monitoring, planning and advocacy to maximize functioning for the population served.
• Coordinates discharge planning through assessment, referral and interdisciplinary planning.
• Manages assigned caseload and adapts to changing priorities in a fast-paced environment.
