Job summary

BayCare is seeking a Hospital Care Coordinator-RN Case Management to support patient care coordination at St Anthony's Hospital in Saint Petersburg, FL. The role is PRN/As Needed with daytime hours and occasional weekend work.

The RN must have 2 years of acute care experience and an active RN license; ACM/CCM certifications are preferred, along with a nursing degree at Associate level (Bachelor’s preferred).

Qualifications

• RN required
• 2 years RN in Acute Care or equivalent
• Associates of Nursing required; Bachelor’s preferred
• ACM and CCM certifications preferred

Responsibilities

• Provides linkage, monitoring, planning and advocacy for our population to achieve and maintain maximum functioning.
• Possesses a clear understanding of the available resources to promote quality cost effective outcomes.
• Effectively maintains and manages assigned caseload, contingent on needs, strengths, abilities, barriers, and preferences of the individual served.
• Coordinates discharge planning efforts through assessment, referral, and interdisciplinary planning.
• Ability to adjust to changing priorities in a fast-paced environment.
