Job summary

BayCare in Florida is seeking a Hospital Care Coordinator - RN Case Management to join a large healthcare network serving the Tampa Bay area. The role focuses on linking, planning and advocacy to support patients toward maximum functioning.

Responsibilities include discharge planning, resource coordination and maintaining a caseload with diverse needs. The candidate should have RN licensure and relevant certifications, with a strong preference for ACM/CCM and acute care experience.

Qualifications

• Must be a Registered Nurse with current licensure.
• At least 2 years in Acute Care.
• At least 2 years of RN experience.
• ACM or CCM certifications preferred.
• Associates or higher nursing degree required; Bachelor’s degree preferred.

Responsibilities

• Provides linkage, monitoring, planning and advocacy for our population to achieve and maintain maximum functioning.
• Promotes quality, cost-effective outcomes by understanding available resources.
• Maintains and manages assigned caseload, based on needs, strengths, abilities, barriers, and preferences.
• Coordinates discharge planning through assessment, referral, and interdisciplinary planning.
• Adapts to changing priorities in a fast-paced environment.
