Job summary

Lifted Hospice seeks a Patient Care Manager (RN) to lead a local hospice team in Austin, ensuring timely admissions, high-quality care, and regulatory compliance. You will guide care plans, mentor staff, and coordinate across nursing, social work, chaplains, CNAs, and volunteers.

In this role, you’ll oversee clinical decisions, support discharge planning, and maintain thorough EMR documentation using Kantime.

Qualifications

• Graduate of an accredited Nursing program (RN).
• Active RN license in good standing.
• CPR certification if required.
• 2 years hospice or home health nursing, leadership experience preferred.
• Excellent communication and clinical judgment.

Responsibilities

• Lead interdisciplinary team including nurses, social workers, chaplains, CNAs, and volunteers.
• Oversee admissions, care plans, and regulatory compliance.
• Mentor and coach clinical staff; support hiring/onboarding.
• Ensure timely documentation and QA across EMR systems.
• Coordinate with physicians and partners; educate patients and families.
• Lead problem-solving and infection control practices.
