Qualifications

• Licensed RN in the state of operation.
• BSN preferred; accredited nursing program graduate.
• Hospice or home care experience required.
• Minimum 2 years of supervisory/management experience preferred.
• Current CPR certification.
• Must be a licensed driver with insured vehicle.

Responsibilities

• Review new patient referrals and assign hospice staff.
• Support and guide clinical staff during visits and case reviews.
• Develop patient care plans and set treatment goals.
• Lead team case conferences for coordinated care.
• Perform chart audits and provide quality feedback.
• Participate in hiring, onboarding, and training.
• Support staff development and continuing education programs.
• Contribute to policy development and professional standards.
• Engage in community outreach and promote hospice services.
• Contribute to quality improvement initiatives and goals.
