Advance Hospice Care is seeking a Nurse Leader to serve as Patient Care Manager in Houston, TX. Reporting to the Executive Director, you will oversee patient care, coordinate visits, and ensure compliant, high-quality hospice services.

The role requires RN licensure in Texas, 3+ years direct patient care with hospice/home health experience, and the ability to lead interdisciplinary teams while maintaining compassionate support for patients and families.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum of 3 years of direct patient care, including 2 years in hospice or home-based care.
• Demonstrated ability to apply hospice principles, industry standards, and regulatory compliance.
• Knowledge and compassion for terminally ill patients and their families.
• Ability to lead a diverse care team while adapting to emotional and clinical needs.
• Strong interpersonal, organizational, and leadership skills.
• Competency in clinical documentation and technology systems.

Responsibilities

• Oversee and direct patient care services to ensure high-quality hospice care.
• Manage visit scheduling, including reassigned, missed, declined, or rescheduled visits based on clinical judgment.
• Triage incoming phone referrals and medical concerns promptly and compassionately.
• Lead or facilitate Interdisciplinary Group (IDG) meetings and document minutes.
• Mentor and train associates, supporting development and compliance.
• Ensure care delivery meets federal, state, and local regulations and company policies.
• Support performance improvement, quality assessments, and operational efficiencies at your location.
