Job summary

Advance Hospice Care is seeking a dedicated Patient Care Manager, RN to lead our hospice team. Reporting to the Executive Director or Administrator, you will oversee patient care activities and ensure high-quality, compliant hospice services.

This full-time role involves directing visits, coordinating care plans, mentoring staff, and ensuring regulatory standards across Medicare/Medicaid guidelines. Previous hospice leadership and EMR familiarity preferred.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum of 3 years direct patient care experience, including 2 years in hospice or home-based care.
• Demonstrated ability to apply hospice principles, industry standards, and regulatory compliance (Medicare, Medicaid, JCAHO, ACHC).
• Knowledge and compassion for terminally ill patients and their families.

Responsibilities

• Oversee and direct patient care services to ensure high-quality hospice care.
• Manage visit scheduling and prioritization based on clinical judgment.
• Triage referrals and medical concerns promptly and compassionately.
• Lead Interdisciplinary Group (IDG) meetings and document minutes.
• Mentor and train staff, supporting development, performance and compliance.
• Ensure care delivery meets federal, state, and local regulatory standards and policies.
• Support performance improvement initiatives and operational efficiencies.
