Job summary

SouthernCare, part of Gentiva, seeks a Patient Care Manager to lead hospice care operations. You will oversee patient care services, ensure regulatory compliance, and mentor staff to uphold high standards across visit planning, triage, and interdisciplinary teamwork.

You will coordinate care plans, facilitate IDG meetings, and drive performance improvements in a compassionate, patient-centered environment.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum 3 years of direct patient care experience, including 2 years in hospice/home-based care.
• Demonstrated ability to apply hospice principles, industry standards, and regulatory compliance (Medicare, Medicaid, JCAHO, ACHC).
• Knowledge and compassion for terminally ill patients and their families.
• Ability to lead a diverse care team while adapting to the emotional and clinical needs of patients from various backgrounds.
• Strong interpersonal, organizational, and leadership skills.
• Competency in clinical documentation and technology systems.

Responsibilities

• Oversee and direct patient care services to ensure high-quality hospice care.
• Manage visit scheduling and prioritize based on clinical judgment.
• Triage phone referrals and medical concerns promptly and compassionately.
• Lead Interdisciplinary Group (IDG) meetings; present updates and document minutes.
• Mentor and train associates; support performance and compliance.
• Ensure care delivery meets regulatory standards and company policies.
• Support quality improvement initiatives and operational efficiencies at your location.
