Job summary

SouthernCare seeks a dedicated Patient Care Manager to oversee clinical operations and lead hospice services, reporting to the Executive Director or Administrator.

You will coordinate patient care, manage visits, supervise staff, ensure regulatory compliance, and drive performance improvement across the location. This full-time role offers benefits and growth within a compassionate team.

Qualifications

• RN license in state of employment
• Graduate of accredited nursing program
• Minimum 3 years of direct patient care, incl. 2 years in hospice/home health
• Hospice/regulatory compliance knowledge (Medicare/Medicaid/JCAHO/ACHC)
• Compassion for terminally ill patients and families
• Ability to lead a diverse care team
• Strong interpersonal, organizational, and leadership skills
• Clinical documentation and tech proficiency

Responsibilities

• Oversee and direct patient care services to ensure high-quality hospice care
• Manage visit scheduling, including reassigned, missed, declined, or rescheduled visits based on clinical judgment
• Triage incoming phone referrals and medical concerns promptly and compassionately
• Lead or facilitate Interdisciplinary Group (IDG) meetings, present updates, review charts, coordinate care plans, document minutes
• Mentor, train, and provide clinical support to associates for development and compliance
• Ensure care delivery meets federal/state/local regulatory standards and company policies
• Support performance improvement initiatives, quality assessments, and operational efficiencies at the location
