Job summary

MJHS Hospice is seeking an experienced Hospice Inpatient Clinical Coordinator in Clifton, NJ to lead hospital partnerships, assess referrals, and provide GIP-level care to inpatients. Qualifications

• Graduate from an accredited School of Nursing; BSN preferred.
• A minimum of two years’ experience in Medical-Surgical or Community Health. Hospice or Oncology nursing experience preferred.
• New York State RN licensure, CPR-BLS.
• Valid NYS Driver's license preferred.
• Familiarity with Microsoft Office applications.
• Ability to work flexible hours to meet the needs of the position, including weekend hours.
• Ability to coordinate the resources and personnel involved in a comprehensive interdisciplinary team environment.
• Ability to work non-judgmentally with patient/family members of any culture, religion, socio-economic background or lifestyle.
• Excellent communication, listening, and organizational skills.
• Ability to speak in public.

Responsibilities

• Develops and strengthens business relationships with hospital partners.
• Responds to requests for information and referrals from hospital personnel and facilitates hospice enrollment.
• Gathers clinical information used in eligibility determinations and ensures compliance with Hospice Conditions of Participation, LCDs, and regulations.
• Coordinates effective symptom management for patients awaiting discharge to home hospice with referring physicians and hospital staff.
• Serves as the RN member of the Inpatient Hospice Interdisciplinary Team (IDT) providing daily coordination of hospice care for GIP patients.
• Develops the hospice nursing plan of care and makes recommendations based on patient/family needs.
• Participates in IDT Meetings and daily GIP Huddle to communicate patient status.
• Coordinates transition plans for patients no longer qualifying for inpatient care.
• Oversees clinical care provided by hospital staff and educates staff on hospice provisions.
• Maintains accurate and current documentation reflecting hospice nursing assessments and interventions.
