Job summary

MJHS Hospice is seeking an Inpatient Clinical Coordinator to lead coordination of General Inpatient (GIP) level care, assess hospital referrals, and strengthen partnerships with hospital partners. The RN will provide specialized hospice care with an interdisciplinary team.

The role requires strong nursing leadership and the ability to develop individualized care plans and coordinate with hospital providers and families. BSN preferred and NJ licensing are important for success.

Qualifications

• Graduate from an accredited School of Nursing; BSN preferred.
• Two years’ experience in Medical-Surgical or Community Health; Hospice/Oncology preferred.
• New York State RN licensure; CPR-BLS.
• Valid NYS Driver's license preferred.
• Familiarity with Microsoft Office applications.
• Flexible hours including weekends.

Responsibilities

• Develop hospice nursing plans of care and recommend services.
• Coordinate with inpatient providers and staff; participate in IDT meetings.
• Maintain accurate and current documentation of assessments and interventions.
• Gathers clinical information for eligibility determinations per Hospice regulations.
• Oversee clinical care provided by hospital staff and educate about hospice care.
