Job summary

MJHS is seeking an experienced Hospice Inpatient Clinical Coordinator to strengthen hospital partnerships, assess referrals for hospice enrollment, and provide General Inpatient (GIP) level of care with compassionate clinical skills. The role requires strong nursing leadership and collaboration within an interdisciplinary team.

Qualifications

• Graduate from an accredited School of Nursing; BSN preferred.
• A minimum of two years’ experience in Medical-Surgical or Community Health. Hospice or Oncology nursing experience preferred.
• New York State RN licensure, CPR-BLS.
• Valid NYS Driver's license preferred.
• Familiarity with Microsoft Office applications.
• Ability to work flexible hours to meet the needs of the position, including weekend hours.
• Ability to coordinate the resources and personnel involved in a comprehensive interdisciplinary team environment.
• Ability to work non-judgmentally with patient/family members of any culture, religion, socio-economic background or lifestyle.
• Excellent communication, listening, and organizational skills.
• Ability to speak in public.
• Ability to demonstrate effective critical thinking skills.
• Ability to solve problems independently.

Responsibilities

• Develops and strengthens business relationships with hospital partners and coordinates admissions to hospice.
• Assess patients referred for hospice enrollment and provide GIP level of care as needed.
• Evaluate patients for hospice and community-based palliative care eligibility and facilitate admissions.
• Collaborate with inpatient providers and the interdisciplinary team to develop individualized care plans.
• Educate referrers, patients and families about hospice services and coordination of care.
• Maintain accurate documentation reflecting hospice nursing assessments and interventions.
