Job summary

MJHS Hospice seeks an Inpatient Clinical Coordinator to strengthen hospital partnerships, assess referrals, and provide GIP-level hospice care with skilled clinical leadership. The role works within an interdisciplinary team to develop care plans and ensure compliant admission decisions.

The ideal candidate is a registered nurse with BSN preferred, NY licensure, CPR-BLS, and experience in medical-surgical or community health settings, with strong communication and organization skills.

Qualifications

• Graduate from an accredited School of Nursing; BSN preferred.
• A minimum of two years’ experience in Medical-Surgical or Community Health. Hospice or Oncology nursing experience preferred.
• New York State RN licensure, CPR-BLS.
• Valid NYS Driver's license preferred.
• Familiarity with Microsoft Office applications.
• Ability to work flexible hours to meet the needs of the position, including weekend hours.
• Ability to coordinate the resources and personnel involved in a comprehensive interdisciplinary team environment.
• Ability to work non-judgmentally with patient/family members of any culture, religion, socio-economic background or lifestyle.
• Excellent communication, listening, and organizational skills.
• Ability to speak in public.
• Ability to demonstrate effective critical thinking skills.
• Ability to solve problems independently.

Responsibilities

• Develops and strengthens business relationships with hospital partners.
• Assess patients referred for hospice enrollment and provide GIP level of care.
• Coordinate admission of eligible patients to hospice in accordance with laws and agency policies.
• Develop individualized care plans with an interdisciplinary team.
• Educate referrers, patients and families about hospice services.
• Maintain accurate and detailed documentation.
