Job summary

MJHS Hospice is seeking an Inpatient Clinical Coordinator in Newark, NJ to lead clinical coordination for GIP level hospice care. You will assess referrals, collaborate with hospital partners, and develop individualized care plans with an interdisciplinary team to ensure seamless patient transitions.

Ideal candidates hold an RN license (NY), BSN preferred, and have experience in medical-surgical or community health settings.

Qualifications

Graduate from an accredited School of Nursing; BSN preferred.
Two years’ experience in Medical-Surgical or Community Health; hospice or oncology preferred.
New York State RN licensure; CPR-BLS.
Familiarity with Microsoft Office applications.
Ability to coordinate resources in an interdisciplinary team.

Responsibilities

Develops strong business continuity relationships to support the agency’s business development goals.
Gathers clinical information used in eligibility determinations and coordinates admissions.
Coordinates effective symptom management for patients awaiting discharge to home hospice.
Serves as the RN member of the Inpatient Hospice Interdisciplinary Team (IDT).
Maintains accurate and current documentation reflecting hospice nursing assessments and interventions.
