Job summary

MJHS Hospice is seeking an Inpatient Clinical Coordinator to lead care coordination for General Inpatient (GIP) services and liaise with hospital partners in New Jersey. The role assesses referrals for hospice enrollment, develops care plans with an interdisciplinary team, and ensures compliant documentation and patient-centered care.

The ideal candidate is an RN with 2+ years in Medical-Surgical or Community Health, holds NYS RN licensure (CPR-BLS), and can work flexible hours including

Qualifications

• Graduate from an accredited School of Nursing; BSN preferred.
• A minimum of two years’ experience in Medical-Surgical or Community Health. Hospice or Oncology nursing experience preferred.
• New York State RN licensure, CPR-BLS.
• Valid NYS Driver's license preferred.
• Excellent communication, listening, and organizational skills.
• Ability to work flexible hours to meet the needs of the position, including weekend hours.

Responsibilities

• Develops strong business continuity relationships to support the agency’s business development goals.
• Responds to information and referrals from hospital personnel and facilitates hospice enrollment for eligible patients.
• Gathers clinical information used in eligibility determinations per Hospice Conditions of Participation and LCDs.
• Coordinates symptom management for patients awaiting discharge to home hospice with referring physicians and hospital staff.
• Serves as the RN member of the Inpatient Hospice Interdisciplinary Team (IDT) for GIP patients.
• Develops the hospice nursing plan of care and makes service recommendations based on patient/family needs.
• Participates in IDT Meetings weekly and GIP Huddle daily for status updates.
• Coordinates transition plans for patients no longer qualifying for inpatient care.
• Oversees clinical care provided by hospital staff and educates on hospice care to direct care personnel.
• Maintains accurate and current hospice nursing assessments and interventions.
