Qualifications

• Graduate from an accredited School of Nursing; BSN preferred.
• Two years’ experience in Medical-Surgical or Community Health; Hospice/Oncology preferred.
• New York State RN licensure; CPR-BLS.
• Valid NYS Driver's license preferred.
• Familiarity with Microsoft Office applications.
• Ability to work flexible hours, including weekends.
• Ability to coordinate resources in a team environment.
• Ability to work non-judgmentally with diverse patients and families.

Responsibilities

• Conduct comprehensive assessments of referred patients.
• Collaborate with inpatient providers and staff on care plans.
• Develop individualized care plans with the IDT.
• Educate referrers, patients and families about hospice services.
• Coordinate with healthcare providers; maintain documentation.
• Oversee transition plans for patients moving from inpatient to home hospice.
