Job summary

TMC HealthCare in Southern Arizona is seeking a Registered Nurse - Case Manager for Outpatient Hospice. This full-time, day-shift role coordinates patient care across disciplines, evaluates outcomes, and maintains patient safety and well-being through individualized plans.

Hospice or Home Health background is desirable; Arizona RN licensure and BLS are required. A $5,000 sign-on bonus is offered, with collaboration from an interdisciplinary team to support patients and families at every step.

Qualifications

• One year of nursing experience in an acute care setting.
• Current Arizona RN licensure and BLS certification.
• Knowledge of nursing process and patient care planning.
• Ability to communicate clearly with multidisciplinary teams.

Responsibilities

• Provides direct patient care and evaluates outcomes.
• Develops and maintains patient care plans; documents accordingly.
• Notifies physicians for unusual events requiring consultation.
• Collaborates with interdisciplinary team to educate patients and families.
• Maintains patient safety and adheres to aseptic techniques.
