Phoenix Hospice in Springfield, MO is seeking an experienced RN Case Manager to join our interdisciplinary team. You’ll provide skilled hospice nursing care, educate families about the care plan, and guide goals-of-care decisions with compassion and clarity.

This role offers full-time and PRN options, a $5,000 sign-on bonus, and strong support including orientation, mileage reimbursement, and comprehensive benefits.

Qualifications

• Active Missouri or Compact RN license required.
• Hospice or long-term care nursing experience preferred.
• Strong assessment, communication and care coordination skills essential.
• Valid driver’s license and auto liability insurance required.

Responsibilities

• Provide compassionate hospice nursing care to patients and families
• Educate patients and caregivers on the hospice process and plan of care
• Guide families through goals-of-care discussions and healthcare decisions
• Conduct initial and ongoing comprehensive assessments
• Perform hospice evaluations and informational visits
• Collaborate with physicians, Hospice Medical Directors, and interdisciplinary team members
• Coordinate care to ensure patient and family needs are fully met
• Communicate changes in patient condition and update plans of care as needed
