Job summary

Copper Sky Hospice is seeking a compassionate Registered Nurse Case Manager to deliver in-home hospice care aligned with physician orders and hospice standards. You will work with an interdisciplinary team to support patients and families, ensuring high-quality outcomes and quality of life.

This role offers a $5,000 sign-on bonus and requires an Arizona RN license, with a BSN preferred and at least one year of nursing experience. Travel to patient homes is expected.

Qualifications

• Graduate of an accredited school of nursing; BSN preferred.
• Minimum of one year of professional nursing experience required.
• Hospice or home health experience preferred.
• Experience caring for aging, chronically ill, or palliative care patients is highly desirable.

Responsibilities

• Assess patients and families to determine hospice care needs.
• Provide skilled nursing care in the home setting using the nursing process.
• Develop and update individualized care plans based on assessments.
• Coordinate interdisciplinary hospice services with the care team.
