Job summary

Northern Arizona Hospice, part of LHC Group within the Optum family, seeks a Registered Nurse Case Manager to deliver the Plan of Care for hospice patients and to collaborate with the care team. You will continually evaluate performance and adjust care plans to promote wellbeing.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• 1+ years of clinical experience.
• Current CPR Certification.
• Valid driver's license, vehicle insurance, and access to reliable transportation.
• Ability to work in diverse home settings with all clients.

Responsibilities

• Make initial nursing evaluation within 48 hours of referral to determine hospice eligibility.
• Identify patient/family needs and re-assess at least every 15 days.
• Initiate and coordinate the plan of care with care team.
• Document problems, goals, interventions, and patient/family responses.
• Collaborate with patient/family, physician, and IDG to provide daily care.
• Instruct and supervise patient/family in self-care techniques when appropriate.
• Maintain accurate clinical notes and strive for quality of care improvements.
