Job summary

UnitedHealth Group's Northern Arizona Hospice is seeking a Registered Nurse Case Manager to deliver the Plan of Care for hospice patients, collaborating with the care team to promote physical, psychosocial, emotional, and spiritual well-being. The role supports a 3x12 or 4x10 schedule to enhance work-life balance and offers a $7,500 sign-on bonus for full-time hires.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• 1+ years of clinical experience.
• Current CPR Certification.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Make the initial nursing evaluation within 48 hours of referral to determine eligibility for hospice services.
• Identify patient/family needs and re-assess as needed, at least every 15 days.
• Initiate and coordinate the plan of care.
• Document problems, goals, interventions, and patient/family response.
• Collaborate with patient/family, attending physician and IDG for daily care.
