Job summary

Heart of hospice, part of the LHC Group and within the Optum family, seeks a Registered Nurse Case Manager to oversee nursing care for hospice patients. You will lead the Plan of Care development and coordinate with the care team to promote physical, psychosocial, emotional and spiritual well-being.

The role emphasizes compassionate care for patients and families facing end-of-life challenges, with collaboration across disciplines to deliver high-quality outcomes.

Qualifications

• Must hold a valid RN license.
• Experience in hospice or palliative care is desirable.
• Ability to develop and update patient care plans.

Responsibilities

• Deliver and manage the Plan of Care for hospice patients.
• Continuously evaluate personal and professional performance for quality of care.
• Collaborate with physicians, nurses and other professionals to implement treatment strategies.

Continuously evaluating personal and professional performance and making necessary changes to increase productivity and quality of care delivered. You\'ll also develop the patient\'s Plan of care in collaboration with other professionals and implement treatment strategies based on scientific nursing theory that promotes physical, psychosocial, emotional, and spiritual well-being. Familiar with the concepts and needs of patients/families who are facing death and dying.
