Job summary

Trustbridge Hospice, part of Empath Health, is seeking an RN Hospice Case Manager in the Boca area. This role focuses on coordinating care, advocating for patients and families, and delivering education to support comfort and quality of life.

You will work Monday–Friday 8:30am–5:00pm with a weekend rotation every six weeks, collaborating with the primary physician and IDG to implement individualized plans of care.

Qualifications

• Active Florida RN license required.
• Current Basic Life Support (BLS) certification.
• One year of experience in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or hospice nursing preferred.

Responsibilities

• Coordinate patient care under the direction of the primary physician and interdisciplinary team.
• Serve as advocate for patients and families, addressing needs promptly.
• Develop and implement individualized care plans as part of the IDG.
• Monitor patient conditions, provide interventions, and deliver education to improve comfort and quality of life.
• Support families throughout the hospice journey and ensure high patient/family satisfaction.

Trustbridge Hospice , a member of Empath Health, is currently seeking a caring and motivated RN Hospice Case Manager to join our team in the Boca area. You would work the following schedule: Monday - Friday 8:30am - 5:00pm with a weekend requirement (One weekend every 6 weeks).

What you’ll Do

As an RN Case Manager, you will play a vital role in coordinating care and supporting patients and families throughout their hospice journey. You will:

• Serve as a key advocate for patients and families, ensuring their needs are recognized and addressed
• Coordinate comprehensive care under the direction of the primary physician and in collaboration with the interdisciplinary team
• Develop and implement individualized plans of care as part of the IDG (Interdisciplinary Group)
• Use your clinical expertise to monitor patient conditions, provide interventions, and deliver education that enhances comfort and quality of life
• Address concerns promptly and thoughtfully to ensure a high level of satisfaction and trust

What You’ll Need

• Registered Nurse currently licensed in the State of Florida.
• Current Basic Life Support Certification.
• One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
• Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
• Basic computer skills and EMR software experience a plus.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
