Job summary

Trustbridge Hospice, a member of Empath Health, seeks a compassionate Hospice Registered Nurse (RN) Case Manager to join our Palm Beach County team. This full-time role features weekday hours with a weekend requirement, integrating patient-centered care with family guidance.

You’ll work with the interdisciplinary team to develop and implement individualized care plans, monitor patients, and provide education to enhance comfort and quality of life.

Qualifications

• Active Florida RN license.
• Current Basic Life Support (BLS) certification.
• One year of experience in public health, home nursing care, oncology, medical/surgical, geriatric, or hospice nursing preferred.
• Understanding of hospice nursing: case management, crisis intervention, communication, teaching, thanatology.
• Proficient computer skills with EMR software experience.
• Reliable transportation with valid Florida driver's license.

Responsibilities

• Advocate for patients and families, ensuring needs are recognized and addressed.
• Coordinate comprehensive care under direction of the primary physician and with the interdisciplinary team.
• Develop and implement individualized plans of care as part of the IDG (Interdisciplinary Group).
• Monitor patient conditions, provide interventions, and deliver education to enhance comfort and quality of life.
• Address concerns promptly to ensure high satisfaction and trust.

Trustbridge Hospice, member of Empath Health, is seeking a caring and motivated Hospice Registered Nurse (RN) Case Manager to join our dedicated hospice team in the Palm Beach County area. In this vital role, you will provide exceptional, patient-centered care while guiding families through one of life’s most meaningful journeys on the following schedule: Monday - Friday 8:30am - 5:00pm with a weekend requirement.

What you’ll Do

• Serve as a key advocate for patients and families, ensuring their needs are recognized and addressed
• Coordinate comprehensive care under the direction of the primary physician and in collaboration with the interdisciplinary team
• Develop and implement individualized plans of care as part of the IDG (Interdisciplinary Group)
• Use your clinical expertise to monitor patient conditions, provide interventions, and deliver education that enhances comfort and quality of life
• Address concerns promptly and thoughtfully to ensure a high level of satisfaction and trust

What You’ll Need

• Registered Nurse currently licensed in the State of Florida.
• Current Basic Life Support Certification.
• One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
• Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
• Basic computer skills and EMR software experience a plus.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
