Job summary

Trustbridge Hospice, part of Empath Health, is seeking a compassionate RN Hospice Case Manager to serve patients and families across Southern Palm Beach County. The role coordinates care, advocates for patients, and works with an interdisciplinary team to develop plans of care.

You must be a Florida-licensed RN with BLS, have at least one year of hospice or related nursing experience, and possess strong communication, teaching, and EMR skills. The schedule is 8:30am–5pm with every other weekend.

Qualifications

• Registered Nurse currently licensed in the State of Florida.
• Current Basic Life Support Certification.
• One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
• Understanding of Hospice Nursing: case management, crisis intervention, communication, teaching, thanatology.
• Basic computer skills and EMR software experience are a plus.
• Adequate transportation with valid Florida driver's license.

• Serve as a key advocate for patients and families, ensuring their needs are recognized and addressed
• Coordinate comprehensive care under the direction of the primary physician and in collaboration with the interdisciplinary team
• Develop and implement individualized plans of care as part of the IDG (Interdisciplinary Group)
• Use your clinical expertise to monitor patient conditions, provide interventions, and deliver education that enhances comfort and quality of life
• Address concerns promptly and thoughtfully to ensure a high level of satisfaction and trust

What You’ll Need
