Job summary

Trustbridge Hospice, a member of Empath Health, seeks a compassionate Hospice RN Case Manager to join our northeast Broward County team in Fort Lauderdale. Mon–Fri 8:30am–5:00pm with weekend coverage, providing patient-centered care and guiding families through the hospice journey.

The role emphasizes coordination with the IDG, individualized care plans, and education to enhance comfort and quality of life. A Florida RN license and BLS are required; hospice experience is preferred.

Qualifications

• Active Florida RN license.
• BSN preferred.
• Hospice certification preferred.
• Current BLS certification.
• Experience in hospice, home health, or geriatric nursing preferred.

Responsibilities

• Coordinate care under physician direction with the interdisciplinary team.
• Develop and implement individualized plans of care for patients.
• Monitor patient conditions and provide clinical interventions.
• Deliver education to patients, families, and caregivers.
