Job summary

Trustbridge Hospice in Florida is seeking a dedicated RN Hospice Case Manager to join our team in Central Palm Beach County. This PRN role runs from 8:30 a.m. to 5 p.m. with weekend availability, supporting patient- and family-centered care within our Empath Health network.

You will coordinate care under the physician and RN, identify needs, and contribute to an individualized care plan within an interdisciplinary group. A BSN and hospice certification are preferred, with BLS current.

Qualifications

• Registered Nurse licensed in Florida is required.
• BSN preferred.
• Hospice certification preferred.
• Current Basic Life Support Certification.
• One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing, or Hospice nursing preferred.
• Understanding of Hospice Nursing concepts: case management, crisis intervention, communication, teaching, thanatology, and needs.
• Basic computer skills and EMR software experience are a plus.
• Adequate transportation with valid Florida driver's license.

Responsibilities

• Coordinate care for the patient/family under the direction of the primary physician and registered nurse.
• Identify patient/family needs and develop an individualized plan of care.
• Work within the interdisciplinary group to implement the plan of care.
• Utilize monitoring and teaching to maximize patient comfort and quality of life.
• Maintain confidentiality and provide compassionate, personalized service.
• Ensure complete satisfaction of patients and families and address concerns promptly.

What you’ll Do

• Support and carry out the organization’s mission, vision, and values on a consistent basis.

• Coordinates the care for the patient/family under the direction of the primary physician and registered nurse.

• Responsible for identifying patient/family needs.

• Coordinating the care for the patient/family.

• As part of the interdisciplinary group (IDG), contributes to and follows an individualized plan of care

• Utilizes monitoring, intervention, and teaching skills to maximize patient comfort and enhance the quality of life for the patient/family unit.

• Pledge to make the comfort and care of patients and families the highest priority.

• Consistently provide a high level of personalized, sincere, and engaging service to patients and families at all times.

• Ensure the complete satisfaction of patients and families; promptly and effectively address all concerns.

• Maintain utmost respect for confidentiality in the care of the patient/family.

What You’ll Need

• Registered Nurse currently licensed in the State of Florida.

• BSN preferred

• Hospice certification preferred.

• Current Basic Life Support Certification.

• One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing, or Hospice nursing preferred.

• Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills, and needs

• Basic computer skills and EMR software experience a plus.

• Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
