Job summary

Trustbridge Hospice, a member of Empath Health, is seeking a compassionate Hospice Registered Nurse (RN) Case Manager to join the team in the northeast Broward County area. The role is Monday–Friday 8:30am–5:00pm with a weekend requirement.

Responsibilities include advocating for patients and families, coordinating care with an interdisciplinary group, and developing individualized plans of care to enhance comfort and dignity at end of life.

Qualifications

• Registered Nurse licensed in Florida.
• BSN preferred.
• Hospice certification preferred.
• Current Basic Life Support Certification.
• 1 year of public health, home nursing, oncology, medical/surgical, geriatric or Hospice nursing preferred.
• Proficient with EMR software and basic computer skills.

Responsibilities

• Advocate for patients and families, ensuring needs are addressed.
• Coordinate comprehensive care under physician direction with IDG.
• Develop and implement individualized plans of care as part of IDG.
• Monitor patient conditions, provide interventions, and educate to improve comfort and life quality.
• Address concerns promptly to ensure satisfaction and trust.

Trustbridge Hospice, a member of Empath Health, is seeking a caring and motivated Hospice Registered Nurse (RN) Case Manager to join our dedicated hospice team in the north east Broward County area. In this vital role, you will provide exceptional, patient-centered care while guiding families through one of life’s most meaningful journeys on the following schedule: Monday - Friday 8:30am - 5:00pm with a weekend requirement.

What you’ll Do

As an RN Case Manager, you will play a vital role in coordinating care and supporting patients and families throughout their hospice journey. You will:

• Serve as a key advocate for patients and families, ensuring their needs are recognized and addressed
• Coordinate comprehensive care under the direction of the primary physician and in collaboration with the interdisciplinary team
• Develop and implement individualized plans of care as part of the IDG (Interdisciplinary Group)
• Use your clinical expertise to monitor patient conditions, provide interventions, and deliver education that enhances comfort and quality of life
• Address concerns promptly and thoughtfully to ensure a high level of satisfaction and trust

What You’ll Need

• Registered Nurse currently licensed in the State of Florida.
• BSN preferred
• Hospice certification preferred.
• Current Basic Life Support Certification.
• One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
• Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
• Basic computer skills and EMR software experience a plus.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
